If you are bringing a minor child, there is a special questionaire that you must pick up in advance and fill out at home. It is a very detailed history and will take some time to complete. Remember to bring it with you. ****************************************************** Bring all medications that you are taking. ****************************************************** Bring proof of insurance and insurance cards or you will not be seen. Bring information about your copayment. Be aware that most plans have a deductible for the year, usually beginning January 1 of each year. Bring proof that your deductible has been satisfied or you will be expected to pay the full amount for your visit. ****************************************************** If you are bringing your child for consultation, bring something like a toy or book to occupy him or her till you are seen. No drinks or food are allowed in Dr. Lynn's Office. Please turn off any cell phone. ****************************************************** If given prescriptions, bring them directly to the pharmacist as soon as possible. Any prescriptions for controlled substances will not be replaced if lost. Use only one pharmacy.
